DISCLOSURE AND RELEASE AND WAIVER OF LIABILITY AGREEMENT

I, the undersigned (client) acknowledge that I have read and understood the contents of this agreement.

1. Creative Lifeflow LLC (“Creative Lifeflow”) makes no medical claims, representations or guarantees regarding any products, diets, or lifestyle changes. It also makes no medical claims, representations, or guarantees regarding the validity of Enzyme Therapy, Lymphatic Enhancement Therapy, Iridology, VIOME, GI Map Stool Testing, Lumen, Brain Reward, Bach Flowers, Nutritional Consulting, Biomat, Ear Coning (Candling) Biofeedback Results, Wellness Suggestions and Ideas, Dutch Hormone Testing, Yoga and Meditation, or Colon Hydrotherapy and does not take any responsibility for anyone using its services to assist themselves or another person with their health.

2. Creative Lifeflow does not diagnose or provide treatment for any disease. Creative Lifeflows’ advice should not be construed as a claim or representation of procedures or products used that may constitute a specific cure or curative.

3. The services and recommendations made by Creative Lifeflow are intended for educational and informational purposes only. Any action taken as a result of a recommendation by Creative Lifeflow is done at the sole discretion of the client. The services at Creative Lifeflow are not intended to replace professional medical advice. Therefore, please consult your doctor before trying any suggestions given by Creative Lifeflow. Creative Lifeflow does not assume responsibility for decisions made by the client regarding their health.

4. Without limitation you understand and agree that Creative Lifeflow is not liable for any direct, indirect, consequential, or incidental damage, injury, death, loss, delay, or inconvenience of any kind which may be occasioned by reason of any act or omission, including, without limitation any willful or negligent act or failure to act, or breach of contract. You agree to be responsible for all legal costs and fees that may result from action(s) on your part or on the part of your representative(s) against Creative Lifeflow.

5. By signing this informed consent you agree to forever release Creative Lifeflow from any and all actions, claims, or demands, including but not limited to illness, injuries, or death that you, your heirs, next of kin, spouse, and/or legal representatives now have or may have in the future related to your participation of Creative Lifeflow services.

6. I am aware that there are no refunds on sessions or packages purchased if your package is expired or on sessions that have already been redeemed; and that I need to give a 24-hour cancellation notice in order to avoid a $30 dollar cancellation fee.

7. I am aware that Creative Lifeflow DOES NOT participate in disability claims.
